Is MLTSS right for you or your loved one?

 

Managed Long Term Services and Supports (MLTSS) provides help for NJ FamilyCare members with long-lasting health problems or disabilities.

 

Long-term care services include help with everyday tasks like bathing, dressing, getting around the house, making meals, or doing household chores. A designated Care Manager will help you find the services you need. They will do an in-home assessment, develop a personal care plan, and coordinate benefits with your other healthcare providers.

 

MLTSS is for individuals in New Jersey who meet:

 

Financial eligibility requirements, including monthly income and assets

 

Clinical requirements (qualifies for nursing home level of care)

 

Age and/or disability requirements

 

Age 65 years or older; or

 

Under age 65 and blind or disabled

Support for behavioral health needs

 

If you’re enrolled in MLTSS, Fully Integrated Dual Eligible Special Needs Plans (FIDE-SNP) or clients of the Division of Developmental Disabilities (DDD) you have access to behavioral healthcare completely through managed care including:

 

Acute partial hospitalization

 

Adult mental health rehabilitation

 

Outpatient mental health services

 

Acute inpatient psychiatric hospitalization

 

Medication Assisted Treatment

 

Partial care/psychiatric partial hospitalization

 

Substance use disorder services — including outpatient, intensive outpatient, partial care, ambulatory withdrawal management, short-term residential, and detoxification services
