Individual and Family Plans

 

In Florida: Coverage provided by Simply Healthcare, Inc. doing business as Wellpoint Florida, Inc.  In Maryland:  Coverage provided by Wellpoint Maryland, Inc.  In Texas:  Coverage provided by Wellpoint Insurance Company.

Group Health Plans

 

For self-funded plans, claims are administered by Wellpoint Life and Health Insurance Company. Insurance coverage is provided by Wellpoint Life and Health Insurance Company.

Medicare and Medicaid Plans

 

Not connected with or endorsed by the U.S. Government or the federal Medicare program.

 

Wellpoint, a Medicare Advantage organization with a Medicare Contract, offers HMO, HMO D-SNP, HMO C-SNP, HMO I-SNP, LPPO, and PDP plans. Wellpoint HMO D-SNP plans additionally contract with state Medicaid programs. Enrollment in a Wellpoint health plan depends on contract renewal.

 

In Arizona: coverage is provided by Wellpoint Texas, Inc., Wellpoint Ohio, Inc., or Wellpoint Insurance Company. In Iowa: coverage is provided by Wellpoint Iowa, Inc. In New Jersey: coverage is provided by Wellpoint New Jersey, Inc. or Wellpoint Insurance Company. In New Mexico: coverage is provided by Wellpoint Community Care of New Mexico, Inc., In Tennessee: coverage is provided by Wellpoint Texas, Inc. or Wellpoint Tennessee, Inc. In Texas: coverage is provided by Wellpoint Texas, Inc. or Wellpoint Insurance Company. In Washington: coverage is provided by Wellpoint Washington, Inc. Part D coverage is provided by Wellpoint Life & Health Insurance Company.

 

 

For Arizona:

 

Medicare Supplement coverage is provided by Wellpoint Insurance Company. Once enrolled into your Medicare Supplement insurance plan, your coverage is guaranteed for the life of the plan with only two exceptions/restrictions: nonpayment of premiums or material misrepresentation.

 

 

For Iowa:

 

Wellpoint Iowa, Inc. is an HMO D-SNP plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in Wellpoint Iowa, Inc. depends on contract renewal.

 

 

For New Jersey:

 

For Plans H3240_013 and H3240_016: Wellpoint Dual Coordination (HMO D-SNP) is a D-SNP plan with a Medicare contract and a contract with the New Jersey Medicaid program. Enrollment in Wellpoint Dual Coordination (HMO D-SNP) depends on contract renewal.

For Plan H3240_024: Wellpoint Dual Secure (HMO POS D-SNP) is a D-SNP plan with a Medicare contract and a contract with the New Jersey Medicaid program. Enrollment in Wellpoint Dual Secure (HMO POS D-SNP) depends on contract renewal.

 

 

For Tennessee:

 

Wellpoint is an HMO D-SNP plan with a Medicare contract and a contract with the State Medicaid program. Enrollment in Wellpoint depends on contract renewal.

For Texas:

 

Medicare Supplement coverage is provided by Wellpoint Insurance Company. Once enrolled into your Medicare Supplement insurance plan, your coverage is guaranteed for the life of the plan with only two exceptions/restrictions: nonpayment of premiums for the life of the plan with only two exceptions/restrictions: nonpayment of premiums or material misrepresentation.

 

For Dual-Eligible Special needs Plans (D-SNP): Wellpoint is an HMO D-SNP plan with a Medicare contract and a contract with the State Medicaid program. This plan is available to anyone who has both Medical Assistance from the State and Medicare. Enrollment in Wellpoint depends on contract renewal.

 

 

For Washington:

 

Wellpoint Washington, Inc. is an HMO D-SNP plan with a Medicare contract and a contract with the Washington Apple Health (Medicaid) program. Enrollment in Wellpoint Washington, Inc. depends on contract renewal.
