Dual Eligible Special Needs Plan (D-SNP)
D-SNPs offer help for people eligible for both Medicare and Medicaid:
- For Medicare, you qualify if you’re 65 or older or have certain health conditions.
- For Medicaid, you qualify based on your income levels instead of age.
Our D-SNPs may include:
- Prescription drug coverage
- Routine dental, vision, and hearing coverage
- Benefits Prepaid Card for over-the-counter items, utilities,1 and groceries1
Chronic Conditions Special Needs Plan (C-SNP)
C-SNPs offer support and care for people living with chronic conditions like:
- Diabetes
- Lung conditions
- Heart disease
- End-stage renal disease (ESRD)
Most of our C-SNPs include:
- Additional benefits tailored to your healthcare needs
- Enhanced care management
- Prescription drug coverage
- Routine dental, vision, and hearing coverage
- Lower out of pocket costs
Institutional Special Needs Plan (I-SNP)
Wellpoint also offers I-SNPs in Arizona. An I-SNP offers support for those in a long-term care (LTC) or skilled nursing facility, or who need intermediate care for 90 days or longer.
You can enroll in an I-SNP if you have Original Medicare and live in one of our network nursing homes or assisted living communities (or other plan-approved location).
