Our Condition Care (CNDC) programs allow you to ask questions about your condition, get benefit information, and connect you with local resources.

Who can join
Washington Wellpoint members with any of the health conditions listed can participate in a CNDC program. Our CNDC case manager will talk with you about your health history, health goals, and work with you to find solutions so you can live well.
Case managers also offer tools for weight management and tobacco cessation (how to stop using tobacco such as quitting smoking).

Health Management: Healthy Families
Healthy Families is a six-month program for children 7–17 years of age who are overweight, obese, or at risk of becoming overweight or obese. Healthy Families includes coaching using motivational interviewing, lifestyle education, and written materials to support member-identified goals. Members can be referred to the program by calling 844-421-5661.
