Need help making meals or grocery shopping? Getting dressed? Visiting your doctor? Call us. As part of your Wellpoint STAR+PLUS MMP benefits, we help you get the services and supports to stay in your home or community.

 

Contact your service coordinator at 855-878-1784 (TTY: 711) Monday through Friday from 8 a.m. to 8 p.m. local time. The call is free.

Your home and community caregivers are part of your team

 

You, your primary care provider, and everyone else who takes care of you make up your service coordination team. You work together to create your care plan and set up the services you need. Together, we all work toward one goal: keeping you healthy.

Get the services you need

 

Your service coordinator will get to know you and will work with your service coordination team to develop a care plan just for you that keeps your goals in mind. Your care plan lists the services you’ll get, how often you’ll get them, and who will provide them. Your service coordinator will also check in with you from time to time. He or she will make sure you’re still eligible for and getting the right amount of services. If you need help, call us.

Already using these services?

 

You’ll get all the same services you qualify for now. Wellpoint STAR+PLUS MMP just makes it easier — now you’ll get them through the same health plan that covers your regular doctor visits and prescriptions.

Need help after hours?

 

Your service coordinator is always the first person you should call. But if it’s after hours and you need help right away, call 24-hour Nurse Helpline at 855-878-1784 (TTY: 711). The call is free. You can talk to a nurse anytime — day or night — even on holidays.
