Is a C-SNP right for you?

 

Wellpoint offers three types of C-SNPs for those living with diabetes, heart disease, lung conditions, or end-stage renal disease (ESRD). These plans tailor benefits to specific needs, helping you stay healthier.

Chronic care

Chronic care C-SNP plans offer support and care for people living with:

  • Diabetes
  • Chronic heart failure (CHF)
  • Cardiovascular disorders

Most chronic care C-SNPs include:

  • Blood glucose monitoring
  • Nutritional education and training
  • Insulin management
  • A diabetes self-management program
  • Foot and wound care
  • A SilverSneakers® fitness program
  • A dedicated care team of endocrinologist (diabetes specialist) and/or cardiologist (heart specialist)

Lung care

Lung care C-SNP plans offer support and care for people living with:

  • Chronic obstructive pulmonary disease (COPD)
  • Asthma
  • Chronic bronchitis
  • Pulmonary hypertension
  • Emphysema

Most lung care C-SNPs include:

  • Special coverage for oxygen supplies and other medical equipment
  • A SilverSneakers® fitness program
  • Care from a primary doctor and a pulmonologist (lung specialist)
  • A dedicated care support team

Kidney care

Kidney care C-SNP plans offer support and care for people living with end-stage renal disease requiring any mode of dialysis.

 

Most kidney care C-SNPs include:

  • Transportation to and from dialysis appointments1
  • Education, self-training, services, and supplies to support dialysis and manage care
  • Allowance for over-the-counter (OTC) health items
  • Care from a primary doctor and a nephrologist (kidney doctor)
  • Nutritional foods tailored to your chronic condition

If you’re not eligible for a C-SNP, we have options to meet your unique needs and budget. If you have both Medicare and Medicaid, you may be eligible for a Dual Special Needs Plan (D-SNP). Or consider a Medicare Advantage plan that has doctor, hospital, and prescription drug coverage all in one plan.

You may qualify for SSBCI if you have a high risk for hospitalization and require intensive care coordination to manage chronic conditions such as Chronic Kidney Diseases, Chronic Lung Disorders, Cardiovascular Disorders, Chronic Heart Failure, or Diabetes. For a full list of chronic conditions or to learn more about other eligibility requirements needed to qualify for SSBCI benefits, please refer to Chapter 4 in the plan’s Evidence of Coverage.
