Do you have these cards? A Dual Eligible Special Needs Plan is a type of Medicare Advantage plan for people with Medicare and Medicaid. With a D-SNP, you get Medicare Advantage benefits such as additional dental, vision, and prescription drug (Part D) coverage. This is on top of everything included in Original Medicare.

When to enroll in a D-SNP
If you’re already enrolled in Medicare, you can enroll in a dual plan the first month you become eligible for Medicaid. If you’re already enrolled in a D-SNP, you may be able to change plans monthly depending on the plan you want to enroll in. Call us to learn more about your options.
Is another Medicare Advantage plan right for you?
If you’re not eligible for a D-SNP, we offer other types of Medicare Advantage Special Needs Plans (SNPs), including Chronic Condition Special Needs Plans (C-SNPs). Or, our other Medicare Advantage plans may offer similar benefits. You just need to be enrolled in Original Medicare to apply.
