Am I eligible for WV Medicaid or WVCHIP?

 

Medicaid

 

You may be eligible for WV Medicaid if you:

  • Meet income requirements.
  • Receive Supplemental Security Income (SSI).
  • Are pregnant.
  • Have children 18 or younger in your household.
  • Have one or more disabilities or are blind.
  • Need long-term care services.
  • Have been diagnosed with breast or cervical cancer and aren’t covered by a health plan.
  • Are an adult age 19 to 64.

 

WVCHIP

 

You or members of your household may be eligible for WVCHIP if you:

  • Have children 18 or younger in your household.
  • Are newly pregnant.
  • Live in West Virginia.
  • Do not meet the WV Medicaid income requirements.
  • Are a United States citizen or qualified noncitizen. Children who are not U.S. citizens must provide verification of their immigration status.
  • Meet WVCHIP income guidelines.

How do I apply for WV Medicaid or WVCHIP?

 


1. Gather important information


When you’re ready to apply, make sure you have the following info:

  • Proof of identity — This could be a:
    • Birth certificate
    • Driver’s license
    • U.S. passport
    • U.S. American Native tribal document
    • U.S. military ID
    • Social security card
    • Legal residency document such as a ‘green card’ (I-551) or an employment authorization card (I-766).
  • Your wages—Having as much info about your income will help your reviewers decide eligibility. Good sources of income proof are:
    • Your most recent federal tax return (if you have it).
    • A bank statement showing direct deposit.
    • A statement from your employer.
    • Dated pay stubs.
  • Number of people in your household—If you claim a dependent on your tax return or could be claimed as a dependent by some else, please let us know. Having a clear understanding of your household can help us better match everyone with their preferred health plan.

2. Check your mail


Your local West Virginia Department of Human Services (DoHS) office will send you a notice telling you if you qualify for Medicaid or WVCHIP. If you’re eligible, you’ll receive a medical ID card with your approval notice. You’ll also receive a packet to help you choose a health plan.

Once you’re approved for Medicaid or WVCHIP, you can choose a managed care plan like Wellpoint by:

  • Visiting the Mountain Health Trust website.
  • Calling Mountain Health Trust at 800-449-8466 (TTY 1-304-344-0015), Monday through Friday from 8 a.m. to 6 p.m., Eastern time.
  • Completing the enrollment form you received in the mail.
