We’re glad you’re here. Changing to a new health plan can bring up a lot of questions and uncertainty. At Wellpoint, we’re here to help you to get the most out of your benefits. Here are some tips to getting started.
Five easy steps to get your plan going
1. Check the mail for your member ID card
You’ll use it to go to your primary care provider (PCP), pick up prescriptions, and for your other medical visits. If it’s been more than two weeks since you joined and you haven’t received it yet, just send us a message on the SydneySM Health app or call 800-782-0095 (TTY 711).
2. Create your online account
It’s quick and easy. You’ll just need your member ID number (from your ID card), your birthdate, and your ZIP code. Once your account is online, you can access your digital ID card, and live chat.

The Sydney Health mobile app makes it easy to find a primary care provider (PCP), pull up your member ID card, and check your health information.
3. Make an appointment with your PCP
Your main provider is called a primary care provider (PCP). Your PCP is listed on your member ID card and your online account. You’ll want to visit your PCP as soon as you can. The sooner you have a checkup, the better your PCP can take care of you.

4. Explore your benefits
We want you to make the most of your benefits and get the best possible care. Learn about your Wellpoint plan, including your no-cost, extra benefits. You can also read your member handbook for benefit details.
5. Complete your yearly Health Risk Screener (HRS)
What you tell us about your health is very important. Your HRS will help us understand your health issues so we can give you the care you need. You may be eligible for extra programs and services based on your HRS. Please reach out on the Sydney Health app if you need help filling out your HRS. Make sure you complete the HRS yearly.
